Hospital Emergency Room Overload Essay

As the name implies, the purpose of hospital emergency rooms is to provide quick and expert medical treatment for urgent and life threatening medical problems. The truth of the matter is that patients seek the ER for a variety of illnesses and injuries. They can range from catastrophic to as minor as a young child running his/her first fever. The downfall to this is that those patients who are truly not suffering from life threatening situations are taking away from those that are. Overcrowding results in longer waiting times to see a physician. Overcrowding also may contribute to decreased protection of a person’s privacy and confidentiality and impaired patient evaluation and treatment. According to an article written by Julie Rovner, she states that, â€Å"97% of ER doctors also said they treat patients daily who have private insurance and primary care doctors, but whose primary care doctors sent them to the emergency room for care due to the patient needing care when the doctor’s office was closed† (Rovner, 2011). Overcrowding has also been linked to more deaths in the emergency room than in the past. One reason is hospitals having to close their doors and rerouting ambulances to other hospitals because their ER is full. Nevertheless, there may be solutions to this ever growing problem. Hospitals can take cost-saving measures to help alleviate the overcrowding issue. They can move stable and non-emergency care patients to less expensive settings such as an observation area or walk-in center staffed by nurses. Creating more after hours urgent care centers where patients can be seen for non-urgent or life or death situations. Providing appropriate clinic facilities and care for patients without insurance. Providing physicians with incentives for accepting Medicare and Medicaid new patients will also alleviate the overcrowding issue. Even though any of these measures could help with overcrowding, and with the Affordable Health Care Act being deemed constitutional, providers don’t see overcrowding in the ER slowing down. Due to the constraints of the U.S. health care system, ER care for patientswith non-urgent conditions will continue and probably increase with the onset of the new health care reform. ACEP President David Seaberg said in a statement in July 2012 that, â€Å"Increasing the number of patients on Medicaid without an equivalent increase in the number of physicians willing to take that insurance will surely increase the flood of patients into our nation’s ERs. And that "he's a very kind man." With this, we can see that Boldwood is regarded highly in the eyes of the country folk. However, when he fell for Bathsheba, he became an emotional wreck and his hay ricks ruined as he didn't collect them in before the storm. By doing this, he is shirking his responsibilities. As Gabriel said, "A few months earlier Boldwood's forgetting his husbandry would have been as preposterous as a sailor forgetting he was in a ship." Boldwood forgetting his hay ricks was a huge clue to how much Bathsheba's marriage had affected him. At the end of the story, he tried to take his own life and was only stopped by his worker Sam. Compared to the dignified and respected Boldwood we saw at the start of the novel, this is a drastic change. Similarly, Troyshirks his responsibilities and drags the rest of the workers down with him as well by practically forcing them to drink. He tells them that "If any of the men show the white feather, let them look else where for a winter's work." By saying this, the men had been left with no choice but to do what he told them to. He shows contempt towards Gabriel's suggestion that the hay ricks should be covered in order to protect them from the rain. In the end Gabriel had to do it all by himself as the workers had been too drunk to work. "He saw at once that if the ricks were to be saved that night, or even the nest morning, he would have to save them with his own hands." The author has shown a contrast in the attitudes if Frank Troy and Gabriel Oak. Clearly, Troydid the worker's harm more than good. Therefore, their working relationship with the workers show their worth or lack of worth. Also, the author makes it clear that the workers are not to blame in this matter as they had been suitably apologetic and embarrassed the day after. Devised self-Evaluation

Our first unit of work in the IGCSE was Devised theatre. Our theme was â€Å"The unexpected† and we chose this title because there is an always unexpected things that happens in our life. Our play is about a person who once had a family, job, and life, but after he got fired he loses everything. Then later on in the story a devil and an angel appears giving him idea of earning money. At last the person choses the Devils way instead of the angel’s way of earning money. The devils way of earning money is to wear his old uniform and pretend to be the manager of the supermarket. Then steal the money and walk away. The plan does not go as expectation. All things went wrong. At last nobody wants to help him, even the angel and devil did not want to help him. He even got someone in jail. Our group used the theme to create a 10 minute play exploring on unexpected events. We went about process by firstly planning the events that is going to happen in our play and then think of things that could go wrong in the story. We first started with the idea of a person who is desperate in need of money. Then we changed it gradually bit by bit, until we are satisfied. Input was shared equally by all members of the group by sharing different ideas and contributing to the group. Each member had contributed something to the group. Some people contributed props, equipment, ideas, and etc. Before the start of the performance I felt really nervous and i also excited about that I was going to perform in front of many parents and peers. It was also the first time I had to perform in the public. I try to overcome these emotions by telling myself not to panic and by staying in role during the performance. In my performance, I portrayed myself as an old granny and a little boy. I tried to communicate this character through the use of my tone, clarity, enunciation, volume, gesture †¦ I thought I was able to effectively use my tone, body gesture, and facial expression to create a sense of humor during the performance. For example, I had a hunched back gesture and a cranky voice, when I was being the grandmother. I stayed in role during the performance. I think I need to improve on having more eye contact toward the audience. I admired Andy’s group mainly of their use of comedy and humor. Their exploration of how they used their vocals, and how they used their body gesture was really outstanding during the performance. In conclusion, I think as a group we work quite well and I think our performance was quite successful, but I think there are some improvements to be done. The targets for our devised theatre are that we have to be more confident on contributing ideas to each other.
